In order to execute proper medical care your contact information, including name, phone number, address and email may be shared with our 3rd party vendors who provide certain services such as: lab services, medication fulfillment services, communication services, shipping services or other medical services as part of your care. These 3rd parties also maintain strict protection of your data.
HOW YOUR PHONE NUMBER IS USED:
Your phone number will only be used for the purposes of communication regarding upcoming appointments or to notify you that you need to make an appointment with the Veritas Clinic. No specific medical information regarding your care will be included in these notifications. In some instances that relate to patient/customer service we may notify you of other aspects of your care via text. For example: if a medication shipment is delayed or lost we may contact your by phone or text to make you aware of the situation. Some notifications are sent via 3rd party vendor who will not sell or use your information for marketing or promotional purposes. The Veritas Clinic also will not use your contact information form marketing or promotional purposes.
HOW YOUR EMAIL IS USED:
Similar to the above, your email will only be used to communicate to you about certain aspect of your medical care. Appointment reminders and reminders to schedule a follow up are sent via email. Other communication may include: test results, answering your questions and miscellaneous correspondence regarding your care. Your email address will not be used for marketing or promotional purposes.
HOW TO OPT-OUT OF COMMUNICATION:
If you wish to no longer receive text or email communication from us, simply respond with “STOP” or “UNSUBSCRIBE” to the number or email from which you received the message. You can also contact us by phone (720-450-1690) or email (info@veritasmensclinic.com) if you no longer wish to receive communication. Once we receive your message, you will no longer receive further messages from us. Please note that if you opt-out of communication it may be very difficult for us to manage your care on a ongoing basis.
CHANGES TO OUR PRIVACY POLICY:
The Veritas Clinic reserves the right to change this policy and our Terms of Service at any time. We will notify you of significant changes to our Privacy Policy by sending a notice to the primary email address specified in your account or by placing a prominent notice on our site. Significant changes will go into effect 30 days following such notification. Non-material changes or clarifications will take effect immediately.
